Here is the SMBSD Safety/Risk Management Resources 

Safety or Hazard Reports:

  •  If you have any safety concerns in your classrooms or on campus, please follow the Link and find the  Safety Hazard/Suggestion Report to fill out.  This goes to the District and brings to their attention and safety or hazardous working conditions. They will likely follow-up on this or make a work order.  

  • If you do not receive feedback or see anything corrected within 5-7 work days, please ask the Custodian to check on the Work Order Notes for any follow-up on the open work order.  

  • If there is still no update or change; reach out to Marisa Philley; Jill Fanello; or Erik Voss for Safety Team Support. 

Workplace Violence Reporting:

  • Workplace Violence means any act of violence or threat of violence that occurs in a place of employment including, but not limited to, the following: The threat or use of physical force  resulting  in injury or trauma; An incident involving a threat or use of a firearm or other dangerous weapon; does not include lawful acts of self defense; and more.  (Please see the WVPP for more inclusions).  

  • Employees are encouraged to report a potential workplace violence incident, threat, or other workplace violence concern to their Site Administrator or Supervisor, and to Patricia Zepeda in HR in writing or by phone call, or to law enforcement. 

  • Patricia Zepeda will ask the employee to complete an Employee and Supervisor Industrial Incident Report (SIPE), which will then be reviewed by HR and Risk Management to address any further action needed.
